IEEE Potential Nominee Form Guidelines

The purpose of the IEEE Medals and Technical Field Awards Potential Nomination Form is to stimulate medals and awards nominations.  It is hoped that use of this simple, easy-to-fill-out form will identify potential nominees for IEEE Medals and Awards, and allow an initial screening and follow-up by the appropriate medal/award committee before the initiation of a formal nomination process.

Filling Out Form

• Be sure to indicate your willingness to serve, as the formal nominator for your candidate should that be requested.  If those reviewing the submitted form deem it appropriate, a more complete description of the candidate’s scientific accomplishments and professional activities will be requested to provide the information necessary to fully evaluate the nomination.  If you do not feel qualified to prepare a full nomination, please indicate someone who you believe is qualified and willing to complete a full nomination.
