The following format is for your guidance in the preparation of this nomination form for the "IEEE Corporate Innovation Recognition." Your nominee may qualify under one or more, or all, of items (5), (6), (7), (8) and (9) described below. Please use Pages 6 and 7, if required, to describe your organization's qualifications.
12. Describe the candidate's contributions to the development and realization of programs, products, and/or services. Identify if these are in application, design, manufacturing, service (or other) engineering. Describe what your measures are for the success of the accomplishments.
13. Describe as specifically as possible, within the description of the recognition (refer to guidelines sheet), the technical contributions, include any specific economic impact these contributions had.
14. Describe how the candidate has raised the level of engineering excellence within his organization. Please include any such achievements outside of the candidate's organization.
15. Describe the accomplishment, directly or indirectly, in elevating the role and prestige of electrical engineering or computer science, and engineers or computer scientists in industry and society.
16. Describe how the candidate through management has provided and encouraged the continuing education, utilization, professional development, and recognition of engineers working in electrotechnology.
